Terms of use

When using "WIT" (referred to as "this facility") operated by Imagawa Shoji Co., Ltd., please strictly observe the following items so that you can use it comfortably. We would also like to ask for your understanding and cooperation so that this facility can maintain a clean and enjoyable atmosphere as a place to connect medical care and a healthy future.

 

1.

This facility is available to those who are not prohibited from exercising by a doctor. High school students or the younger need the permission of their parents.

 

2.

Please refer to our website for the usage time of this facility.

 

3.

The facility will be closed during the year-end and New Year holidays, temporary closure in the case of special inspections and maintenances, natural disasters, and other unavoidable circumstances.

 

4.

This facility does not keep cash or valuables, so please keep it at your own risk. Please note that the facility will not be held responsible for any theft, assault or other accidents.

 

5.

The following items are prohibited at this facility.

1. Eating, drinking and smoking outside the designated area

2. Training under the influence of liquor.

3. Facility use for people with wounds, skin diseases, and infectious diseases

4. Bringing in dangerous goods and pets

5. Gambling, solicitation, sales, and similar acts that cause inconvenience to other users

6. Unauthorized photo / video shooting, recording, etc.

7. Requests for unauthorized questionnaire cooperation, etc.

8. To slander or abuse other users

9. Violence or intimidation against other users

10. Acts those are offensive to public order and morals, such as molestation, peeping, and exposure

11. The act of scribbling or making features in the facility

12. Acts of violent words or loud voices when using this facility

13. The act of intentionally making noise, such as dropping an instrument on the floor on purpose

14. Entry into the exclusion zone

15. Business interference acts

16. Unauthorized change of various equipment settings (including but not limited to air conditioning, lighting, sound, etc.)

17. Solicitation of the staff and related persons of this facility to retire from this facility, employment placement to other companies, withdrawal and other similar acts

18. Any other business interference acts

 

6.

Product sales, commercial activities, distribution of leaflets, etc., and specific religious and political advertising activities without the approval of the facility are strictly prohibited in and around the facility.

 

7.

Any kind of gift or present to our staff is strongly declined.

 

8.

Please note the following when using the changing room and lockers.

1. Please use the changing rooms and lockers without causing any inconvenience to other users.

2. Please put your belongings in the designated locker.

3. When you change clothes and leave the changing room, please be careful not to forget anything in the changing room.

4. In particular, make sure you have no leftovers such as watches, rings, earrings, hairpins and other small items.

5. Please do not use headphones, music players, portable radios, or mobile phones that may disturb other users near the changing room or lockers.

6. If there are any inadequaces in the changing room or locker, please inform the staff.

7. Please dispose of the trash in the designated bin.

8. Please do not touch equipment such as changing rooms and locker lighting and air conditioning. If you have any inconveniences regarding lighting or air conditioning, please contact the staff.

9. It is strictly prohibited to store dangerous goods, pets, fresh foods, noise or smelly things in the locker.

10. Please do not leave your shirts and towels wet in the room.

11. The dressing table is for all users. Please keep it clean.

12. If you feel sick in the changing room, please contact the staff immediately.

Please note the following when using the training area.

1. Please use the trainer area without causing any inconvenience to other users.

2. When using the training equipment, check the precautions and pay attention to safety.

3. Please do not eat and drink in the training area.

4. If you find something wrong with the training area, please let the staff know.

5. Dispose of trash, small items, and discarded items in the designated bin.

6. Please do not touch equipment such as lighting and air conditioning in the training area. If you have any inconveniences regarding lighting or air conditioning, please contact the staff.

7. If you feel sick in the training area, please contact the staff immediately.

 

10.

Please note the following when using the conditioning area.

1. Please use the conditioning without causing any inconvenience to other users.

2. Please do not eat and drink in the conditioning area.

3. Please do not touch equipment and goods in the conditioning area without the permission of the staff.

4. If there is something wrong with the conditioning area, please let the staff know.

5. Dispose of trash and discarded items in the designated bin.

6. Please refrain from operating equipment such as lighting and air conditioning in the conditioning area. If you have any inconveniences regarding lighting or air conditioning, please contact the staff.

7. If you feel sick in the conditioning area, please contact the staff immediately.

 

11.

Please note the following when using a private room.

1. Please use the private room without causing any inconvenience to other users.

2. Please do not use the equipment and goods in the private room without the permission of the staff.

3. If you find something wrong with your private room, please let the staff know.

4. Please dispose of trash, small items, and discarded items in the designated bin.

5. Please do not touch equipment such as lighting and air conditioning in private rooms. If you have any inconveniences regarding lighting or air conditioning, please contact the staff.

6. If you feel sick in your private room, please contact the staff immediately.

Membership agreement

 

1. Definition

1. "Member" is a general term for those who use this facility.

2. "Monthly member" means a person who uses this facility with a monthly membership fee system.

 

2. Name and Location

This facility is called "WIT" (referred to as "this facility") operated by Imagawa Shoji Co., Ltd. This facility is located on the 2nd floor of Kirinoha Mall, 2-1-18 Amakubo, Tsukuba City, Ibaraki Prefecture.

 

3. Operating entity

This facility will be managed and operated by Imagawa Shoji Co., Ltd. (6206 Yatabe, Tsukuba City, Ibaraki Prefecture).

 

4. Purpose

The purpose of this facility is to improve the QOL of members and to improve their conditioning and competitiveness between the medical field and daily (competition) life.

 

5. Membership, etc.

1. Members shall be those who agree with the purpose of this facility and accept this agreement and groups, and those who are not prohibited from exercising by doctors for the use of this facility by members (however, If this facility specifically permits it, then it is not the case). Those who fall under any of the following items cannot join.

(1) Those who are closely related to any forms of gangster.

(2) Those who threaten the order and safety of civil society, such as associate members of gangsters, companies affiliated with gangsters, and general assembly houses, or those who have a close relationship with such organizations.

(3) Those who have behaviors, appearances (including tattoos) that are considered to fall under the previous two items.

2. Those who are high school students or younger need the permission of their parents.

 

6. Type of Member

1. The types and rights of members of this facility are subject to the separately determined price list.

2. If necessary, we may set a new member type or abolish it. In this case, the facility shall notify in advance on the facility's homepage, in writing, or on the bulletin board in the facility, and the members shall not object to this.

 

7. Enrollment Procedure

When joining the facility, please specify the necessary items on the prescribed membership application form, etc., and follow the procedure with the membership fee, etc.

 

8. Other Procedures

1. When changing the type of monthly membership, you must complete the procedure by the prescribed method.

2. If there is a change in the contents described when the member applies for membership (address change, etc.), the change procedure must be completed promptly. In this case, the change notification will take effect upon the completion of the change paperwork of this facility.

 

9. Monthly Membership Fee and Other Service Fees, etc.

1. Monthly membership fee and other service fees will be paid by the prescribed method specified by the facility.

2. When withdrawing the membership fee, if the withdrawal is not possible due to an inadequate account or insufficient balance, in principle, the withdrawal will be automatically made in the next month, including the unpaid amount.

3. Members must pay the designated membership fee until they withdraw, regardless of whether they use the facility or not.

4. In principle, monthly membership fees and other service fees will not be refunded.

 

10. Monthly Membership Card

1. Membership card will be issued only to monthly members.

2. Monthly members are required to show their monthly membership card when using this facility.

3. The monthly membership card is valid only for the person himself / herself, and the monthly member cannot transfer, lend or set other security interests to a third party.

 

11. Group Session Coupon Ticket

1. Group sessions can also be purchased with coupon tickets.

2. As a general rule, the coupon ticket you purchased cannot be transferred or resold to another person.

3. In principle, once the fee is stored, it will not be refunded regardless of whether it is used or not.

 

12. Recess System

There is a recess system for members of this facility. Monthly members may use this system when they are unable to use the facility for a long period of time due to injury or illness.

1.The period of suspension is one month, up to a maximum of three months.

2.During the period of suspension, the member will be required to pay the suspension fee for each month.

3.If you wish to use the recess system, you must apply by the 10th of the month, and it will start from the month following the month you apply.

4. To use the recess system, you must submit the prescribed recess form.

5.The membership will be automatically reinstated after the recess period is over.

 

13. Monthly Membership Withdrawal Procedure

When withdrawing from a monthly membership, you must return the monthly membership card and submit the prescribed withdrawal form.

 

14. Disqualification of Monthly Membership

1. If a monthly member falls under any of the following items, the monthly member will naturally lose his / her monthly membership qualification. In this case, the monthly membership card will be invalid and will be returned immediately.

                  (1) Those who have not paid the monthly membership fee for 2 months or more (If the monthly membership fee is delinquent for 2 months, it will be treated as withdrawal. However, the delinquent fee will be judged as an unpaid fee and will be charged) .

                   (2) When the membership fee set by this facility is not paid by the method and deadline specified by this facility.

                    (3) Those who have damaged the honor of the facility and disturbed the order.

                    (4) Those who have made a false declaration at the time of enrollment.

                    (5) When you die (in the case of a corporate member, when you file a petition for dissolution or bankruptcy).

                    (6) Those who have performed other acts those are not suitable for the purpose of this facility.

2. If the monthly member falls under any of the following items and does not accept the request from the facility, the monthly member will lose the monthly membership qualification. In this case, the handling of the monthly membership card will be the same as the previous paragraph.

(1) Those who fall under any of the items of Article 5, Paragraph 1.

(2) Those who violate the membership agreement, terms of use, and other rules of the facility.

 

15.Compensation for Damages

1. This facility does not take any responsibility for personal or physical accidents that occur to members or third parties when using this facility.

2. If a member causes damage to the facility or a third party when using the facility, the member shall promptly take responsibility for the compensation.

3. However, if the cause of the accident or damage is clear, due to the negligence of the facility or the inconvenience of the equipment, the preceding paragraphs do not apply.

 

16. Theft

The facility will not be liable for any damages caused by theft caused by the member's use of the facility.

 

17. Lost / Forgotten / Found Stuff

1. The facility will not be liable for any damages caused by the loss caused by the member's use of the facility.

2. Lost and found items will be dealt with according to the "Kirinoha Mall Found Items Handling" business flow.

 

18. Abolition of Facilities, Equipment and Services and Restrictions on Use

1. Personal session and group session users shall be able to use the facilities and training equipment with priority over facility rental and monthly membership. In addition, the facility shall not be liable for any troubles between members that occur at that time.

2. In the event of a natural disaster, enactment, revision or abolition of laws, administrative guidance, significant changes in social conditions, or other unavoidable reasons, this facility will abolish all or part of the facilities, equipment, and services, or use them. You can limit it.

3. When remodeling, remodeling, maintenance, etc. of facilities / equipment, or when it is deemed necessary for management, this facility will abolish all or part of the facilities / equipment and restrict its use and service provision. The notification will be made on the facility's website, in writing, or posted on each facility.

4. In addition to the above two items, each facility may be closed after prior notice if it is unavoidable due to facility management. As a general rule, this notice will be made on the facility's website, in writing, or posted on the facility, but this does not apply to temporary closures due to unavoidable circumstances.

5. In the case of each of the preceding paragraphs, the member shall not be able to make any claim for damages, etc. to this facility.

 

19. Monthly Membership Fee and Other Service Fees, etc.

This facility can change the fees, etc. that members should pay based on this agreement due to various circumstances. In this case, the facility shall notify the contents on the facility's homepage, in writing, or on the bulletin board in the facility at least one month in advance.

 

20. Collaboration with Tsukuba University Hospital

This facility will cooperate with Tsukuba University Hospital (Orthopedics, SMIT (Tsukuba Sports Medicine and Health Science Center), etc.). Therefore, we may provide information such as the physical condition of members and the content of sessions to Tsukuba University Hospital. In addition, a separate agreement will be set up.

 

21. Terms of Service

1. Terms of use for this facility will be established separately.

2. Members are obliged to comply with the terms of use.

 

22. Agreement Jurisdiction

The Mito District Court Tsuchiura Branch shall be the exclusive agreement jurisdictional court of the first instance for all proceedings related to this agreement.

 

23. Issuance of Terms

1. This agreement will be effective from June 3, 3rd year of Reiwa.

2. This agreement may be revised from time to time as necessary. In this case, as a general rule, we will notify you by the facility homepage, in writing, or by posting in the facility. If revised, its effect shall be extended to all members and the new rules shall be complied with without objection.
